Housing Accommodation Request

 

Use the form below to submit your housing accommodation request to Housing Services for review. In order to receive a Housing Accommodation, you must have documentation supporting the requested accommodation with Disability Support Services (DSS), the Counseling Center (CC), and/or Student Health Services (SHS). Accommodations are made on a needs-based and space available basis. When offering spaces for Housing Accommodations a student's needs will be met; however, their preferences, while considered, are not guaranteed. Please note: All campus residence halls are air-conditioned. More information on the housing options is available on the Housing Services website.  In order for the housing accommodation request to be considered, the student must have documentation on file in DSS, CC, and/or SHS.

All students, regardless of disability status, are expected to pay the current rate of the room they are assigned. The accommodation is the assignment, not the charging of a differential rate.

Audio/Visual Alarms

Will you require audio or visual alarms for emergency egress in your individual room? Please note that audio/visual alarms are standard in the common areas of the residence hall?

If yes, are you able to care for the needs of the animal independently?

Do you have documentation on file with the appropriate office that specifically addresses your need for the requested housing accommodation? If no, please contact Disability Support Services and if necessary also the Counseling Center and/or Student Health Services to submit the appropriate documentation. Please note: just being registered with one of these offices may not be sufficient. The documentation needs to discuss this specific issue.
